Het is door de FDA goedgekeurd voor twee hoofdindicaties: behandeling van <strong>postherpetische neuralgie (PHN)<\/strong> bij volwassenen \u2014 de aanhoudende brandende, stekende zenuwpijn die maanden of jaren na een gordelroosuitbraak kan aanhouden \u2014 en als <strong>aanvullende therapie<\/strong> bij de behandeling van <strong>parti\u00eble aanvallen<\/strong>, met of zonder secundaire generalisatie, bij volwassenen en kinderen van 3 jaar en ouder met epilepsie.<\/p>\n<p dir=\"auto\"><strong>Hoe werkt Neurontin 400 mg?<\/strong> Gabapentine bindt aan de \u03b12-\u03b4-subeenheid van spanningsafhankelijke calciumkanalen in het centrale zenuwstelsel. Dit vermindert de afgifte van exciterende neurotransmitters, waardoor de abnormale zenuwactiviteit die verantwoordelijk is voor epileptische aanvallen en neuropathische pijn afneemt. Het bindt niet rechtstreeks aan GABA-receptoren, ondanks de structurele gelijkenis met de neurotransmitter GABA.<\/p>\n<p dir=\"auto\"><strong>Veelvoorkomende toepassingen<\/strong>:<\/p>\n<ul dir=\"auto\">\n<li>Verlichting van zenuwpijn na gordelroos (postherpetische neuralgie)<\/li>\n<li>Aanvullende behandeling voor parti\u00eble aanvallen bij epilepsie<\/li>\n<li>Soms wordt het off-label voorgeschreven voor andere neuropathische pijnaandoeningen of het rusteloze benensyndroom (onder medisch toezicht).<\/li>\n<\/ul>\n<p dir=\"auto\"><strong>Dosering en toediening<\/strong> Bij postherpetische neuralgie begint de behandeling vaak met een eenmalige dosis van 300 mg op dag 1, verhoogd naar 600 mg\/dag op dag 2 en 900 mg\/dag op dag 3, waarna de dosis wordt verhoogd tot een maximum van 1800 mg\/dag (of hoger in sommige gevallen), verdeeld over drie doses. De capsules van 400 mg maken een flexibele dosering mogelijk. Bij epilepsie bij pati\u00ebnten van 12 jaar en ouder is de startdosis doorgaans 300 mg driemaal daags, zo nodig verhoogd tot 600 mg driemaal daags. De dosering moet worden aangepast bij pati\u00ebnten met een verminderde nierfunctie. De capsules moeten in hun geheel met water worden doorgeslikt en kunnen met of zonder voedsel worden ingenomen. Het maximale interval tussen de doses mag niet langer zijn dan 12 uur.<\/p>\n<p dir=\"auto\"><strong>Kwaliteitsborging<\/strong> Neurontin 400mg capsules zijn oranje, harde gelatinecapsules met de opdruk \"PD\" op de capsulewand en \"Neurontin\/400 mg\" op de dop. Regelmatige controle door een zorgverlener is essentieel, met name bij het starten van de behandeling of bij het aanpassen van de dosering.<\/p>\n<p dir=\"auto\">Als u last heeft van aanhoudende zenuwpijn na gordelroos of ongecontroleerde parti\u00eble epileptische aanvallen, kan Neurontin 400 mg effectieve verlichting bieden als onderdeel van een uitgebreid behandelplan dat ook andere therapie\u00ebn kan omvatten.
